Job Description

As ELV Nurse at our client in Veldhoven, you play a crucial role in the care process for frail elderly people living at home.
You are specialized in short term first line care (ELV), where you perform triage from the care center for general practitioners and hospitals.
With your knowledge you determine where the client or patient can best receive the required care.
You arrange the entire process from A to Z, working independently but always falling back on your colleagues.

What do we ask of you?

• At least MBO level 4 trained in healthcare with a valid BIG registration;
• Current knowledge of home care, inpatient and outpatient care, and familiarity with laws and regulations;
• Experience in nurse triage and affinity for care mediation, client consultation or transfer agency experience is a plus;
• Good communication skills;
• Stress resistant and steadfast;
• You are digitally savvy;
• Upon employment, you can provide us with a Certificate of Good Conduct (VOG);
• Last but not least: you have humor!
